Collect dataRead the symptoms: What did you expect? What didn’t you?


 Has something changed recently? What is unique about this system? Eyewitness statements can describe how the machinery was behaving up until the failure.


 to turn it into information. Turn the information into as many hypotheses as possible. Scrutinize your hypotheses critically. Are they consistent? Do they correspond with data? Do they explain the failure 


Solve the problem by using your hypothesis to evaluate and choose the right solution. Verify that the solution is reliable and durable.


Capture the knowledge learned to help predict and prevent future failures. A holistic approach views the failure as a chance to learn more about the machinery.
